Senior Mental Health Nurse - SW Bury South IDT Adult and Enhanced Community Team

This is a fantastic opportunity for a Mental Health Nurse that is motivated to move forward with their career path. If you are currently in a Band 5/6 position and have at least 24 months post registration experience this may be the post for you.

We are looking for a Mental Health Nurse who is keen to contribute to innovative thinking and service development. You'll be well supported by the MDT.

As with many services we currently use a blended model of virtual and face-to-face working which provides opportunities for flexibility. You will have knowledge, skills and experience of working across the range of complexity and severity in adult mental health. We are looking for individuals who bring enthusiasm, creativity and energy to work in this vital area.

As a team we also pride ourselves on working on a 'needs led' approach, providing a range of psychologically based interventions that support our services users along their recovery pathway.

Main duties of the job

You will undertake the planning/delivery of direct patient care with minimal supervision, ensuring continuity of practice and working within a MDT/multi-agency environment.

  • Managing a caseload and the provision of mental health care services to people with mental health care need.
  • Provide planned nursing care/risk assessment / screening for patients within their care plan andongoing evaluation of patients' needs and identify suitability for potential discharge in conjunction with the Senior Nurse, via caseload management/supervision.Initiate and attend patient centred reviews, case conferences and other meetings as required.
  • To advise the Responsible Medical Officer(s) and/or health and social care professional of any change in the patients' conditions and / or circumstances.
  • To work within the MDT and to form working partnerships with other care providers.
  • To ensure service users/carer(s) are enabled and empowered to make informed decisions about their care and delivery of that care, working with agencies from within and outside the MDT.
  • To provide caseload management supervision to junior staff under direction and supervision of the Senior Nurse.
  • Assist with the supervision of peers if requested.
  • To enable patients to gain and maintain independence.
  • Identifying personal learning and training needs through KSF, appraisal and personal development plans on an annual basis.
  • Undertake the supervision of students that will be undertaking placements within the Team.
