Job summary

Senior Community Mental Health Nurse

This is an exciting opportunity to join the Older Person's services.

We provide a service for older people in need of intensive support with the aim of avoiding hospital admission. We work with people with functional needs as well as those with dementia and experience in both areas would be an advantage.

We are looking for someone with strong communication skills and the ability to engage with clients and carers, often in difficult and distressing circumstances. You will have experience of working in the community and be confident in the assessment and management of risk.

The team has a strong person-centred approach, undertaking holistic assessments and working with clients and carers to achieve positive outcomes for both. The team works closely with statutory and voluntary organisations to achieve these aims and prevent unnecessary admissions to hospital and premature moves to long term care.

Main duties of the job

  • Initial assessments including functional abilities, and risks
  • Development and delivery of treatment plans in conjunction with the service user and carer.
  • To cover duty desk, providing support and advice to service users and other agencies.
  • Liaison and joint working with other professionals
  • To take on management and clinical supervision role
  • To represent the team at meetings with statutory and voluntary sector organisations
  • To be a role model and leader within the team
