Job summary

We have recently been commissioned to pilot a new community outreach team which aims to support Children and Young People (CYP) under the age of 18 who are reaching crisis and at risk of repeat presentations to acute hospitals / A&E services or who may otherwise require an admission into an inpatient facility.

The aim of this new pilot team and the wider system is to prevent CYP from being Inpatient admissions wherever possible, and to support them in the community. The team will serve as a gateway in and out of Tier 4 (T4) provision so CYP will be able to access T4 provision / support without the need for admission. This is a significant change in both the clinical offer and culture.

We have an exciting opportunity for Mental Health Practitioners / Nurses / Social Workers / Occupational Therapists to join our new Outreach Team, working alongside the existing clinical teams across the newly established Specialist Services Care Group which is committed to supporting staff helping families and young people thrive.

The newly established Specialist Services Care Group provides inpatient and community services for children and adults needing specialist mental health assessment, care, and treatment. Main duties of the job

The Mental Health Practitioner role offers a wonderful opportunity to be part of the multidisciplinary leadership team, influential in shaping, developing, and delivering care to meet the needs of the local population, in partnership with all other stakeholders. We are looking for individuals who have enhanced clinical skills and experience with an ability to inspire, motivate and deliver clear improvements for patients and staff.

Whilst experience in working with children and young people would make you perfectly suited to this role, we are really open to welcoming people from clinical backgrounds who have experience in other areas of mental health. We understand that you will be able to bring with you a wealth of skills and experience to the team and would welcome ways of thinking.

It will be quite a challenging environment at times, so we need clinicians with a positive attitude, compassion, resilience, and a hands-on approach. It is vital that the clinician is reflective around issues of power and difference, see the importance of working across systems, and can do that effectively. We can accommodate some flexible working in line with the service need. There will be a requirement to be able to travel independently.
