Job summary

Are you a Newly Qualified Nurse with mental health experience looking to start your career in an Adult Community Mental Health Team that supports robust induction, preceptorship, coaching, mentoring and continuing professional development?

The Adult Community Mental Health Team based at Chatterton House, Kings Lynn, are excited to welcome applications from Registered Nurses with experience or keen interest in working with complex mental health conditions.

We are keen to support development opportunities and pathways, such as:

  • An in-house Preceptorship Programme for Newly Qualified Practitioners
  • Developmental Band 5 to Band 6 roles for Registered Practitioners
  • Band 6 Development Programmes (such as Leading Confidently)
  • Advanced Practice opportunities such as Non-Medical Prescribing (NMP) and Advanced Nurse Practice (ANP) which has enabled clinicians to progress to Nurse Consultant and Clinical Director roles
  • Funding available for training programmes such as Structured Clinical Management (SCM) and Mentalisation Based Therapy (MBT)

Main duties of the job

Responsible for the joint assessment, planning, implementation and evaluation of service user care packages, in consultation with the Care Co-ordinator and within the context of the Care Programme Approach

To work towards and participate in the model of care appropriate to area i.e. the social inclusion and recovery with the service user. To identify any carers involved and ensure statutory obligations are met.

The post holder will respond and adapt to service developments and demands at the direction of the senior nurse (band 6 or above).

The post holder will be an active participant in education and training activities in the clinical area with reference to both pre and post registration learning.

The post holder will be responsible for a delegated case-load with support from the senior nurse (band 6 or above).

The post holder will be an effective team member and maintain an open dialogue with all individuals involved in supporting clients within the community.

The post holder will be a competent practitioner in medicine management and contribute to local audit identified by the Trust governance process.

As a qualified practitioner the post-holder will be personally accountable for their practice in accordance with the "The NMC Code of Professional Conduct: Standards for Conduct, Performance and Ethics"

As named nurse, to ensure accurate mental health clustering is carried out on admission, discharge and at reviews.
