Our Waveney Adult Community Mental Health Team are seeking a dedicated Community Mental Health Practitioner to join their team.

Our collaborative team are based at Victoria House, Lowestoft, but travel across all of Waveney to visit service users. You'll be working alongside a close knit multi-disciplinary team of Assistant Practitioners, Social Workers, Nurses, Occupational Therapists, Peer Support Workers, Psychologists, Psychological Therapists, Psychiatrists and Care Support Workers.

We provide a community-based service to Lowestoft and rural Waveney market towns and villages; we have excellent working relationships with external agencies and partners who are all working together to develop recovery-based care plans.

We're a relatively new, developing team and are looking for individuals with experience working in the community and with a knowledge/understanding of assessment, the referral process, case handling and working collaboratively with the wider MDT.

Our expectation is for you to join us as a Community Mental Health Practitioner and progress into a senior role within agreed time frames by achieving set competencies.

Main duties of the job

The ideal candidate will be a qualified professional Nurse, Occupational Therapist or Social Worker with experience of working in the community supporting working age adults with mental health problems following the Care Programme Approach.

We'd like you to be able to demonstrate a sound understanding of your role as a Care Coordinator and have an understanding of the triage and assessment process, as we accept a number of referrals from GP surgeries and other services.

You'll need to demonstrate robust and sound assessment skills; our secondary mental health service cares for individuals with severe and enduring mental health problems such as bipolar, trauma, personality disorder and schizophrenia.

We're always developing new ways of supporting our service users; we boast a thriving Recovery College and Therapeutic Day service, plus access to respite and admission prevention support within a supported housing provider.

Team members are regularly involved in whole service delivery development, including Day service, Psychological Therapies and joint working with Primary Care.
