At Wedgwood House, we have exciting opportunities for an existing or aspiring band 6 nurse to join the Bed Management team. If you have the qualities described above then we want you to think about coming to be part of our Wedgwood family and join us.
Main duties of the job
As part of our ongoing strive for excellence we are developing a 'care team' approach patient centred care. The Discharge Co-ordinator will be responsible for supporting patient discharges for all of our acute wards. The successful applicant will work closely with hospital clinicians, social work teams, personal care services, Julian support staff and multi-disciplinary team members to deliver a high quality, patient focussed service.
About us
Wedgwood House is a purpose-built mental health unit comprising 2 acute adult mental health inpatient wards, an older person's inpatient ward, as well as crisis, home treatment and psychiatric liaison services.
The role is an excellent opportunity to support our service users journey, whilst developing key skills and relationships across the acute and community services.
