We have a vacancy for a full time Band 6 Community Mental Health Nurse to work in our Waveney Older Persons Community Team, based at Carlton Court Hospital, Lowestoft.
We are expanding our team and looking forward to a new nurse to join us.
The area we cover is the Waveney and Lowestoft area a total of 13 GP practices, we are flexible in working practice with staff working from home or in the office.
We are a small and busy team, ideally you will have experience of managing your own caseload .
Our dedicated team supports patients with dementia of all ages, and those over 75 with organic and functional MH presentations. We're multi-disciplinary with a variety of skills and experience. When you join us, you'll have the opportunity to work alongside a range of teams both in and outside NSFT, such as Social Services, In reach Out Reach, DIST Care Homes and Care Agencies.
Main duties of the job
As Community Mental Health Nurse you will be responsible for:
- Holding and managing a caseload of patients with organic and functional presentations under CPA & NCPA - assessing and planning treatment for them
- Working with the patient/their carers to plan care and optimise treatment plans
- Attending MDT meetings, feeding back to the MDT and working within the team for the best outcome for the patient
- Administering depot medication
- Working independently with minimal supervision, within professional boundaries
- Duty working triaging referrals via AAT
- Attending 117c meetings, discharge meetings from inpatient wards
