Job summary

Band 5 Community Dietitian

We are looking for a motivated and enthusiastic Dietitian to join our friendly team of Community Dietitians and Dietetic Assistant Practitioner

The post will suit a dietitian who thrives on variety and wishes to experience patient care in arrange of different settings (clinics/ home visits/care homes and ward based intermediate care)

You will maintain skills in a range of clinical areas including nutrition support, gastroenterology, liver and pancreatic conditions, diabetes, and other long-term conditions.

You will enjoy managing patients holistically in their community setting, liaising with acute dietitians, tertiary centres and other health professionals as needed.

You will be an excellent communicator enjoying both 1-1 consultations and telephone consultations and enjoy working with other professionals on the inpatient intermediate care unit

If successful you will join a friendly team with 9 dietitians, a dietetic assistant practitioner and clerical support based within easy access of Manchester and the peak district

As part of the Community Dietetic Service, you will assess, implement and monitor nutritional care plans for adult patients referred to the service.

You will provide a service for patients in a range of locations including clinics (face to face and via phone), patients in their own homes, care homes, and on the ward based intermediate care unit.

You will be interested in maintaining a wide range of dietetic skills You will be involved in initiatives to ensure the appropriate prescribing of nutritional supplements and will participate in training on malnutrition including screening and first line interventions as needed.

You will be involved in the training of student dietitians during their placements as needed
