Job summary

IUCT Nurse - Band 6 - Full time - Fixed Term - 12 Months

This is an exciting opportunity for a skilled nurse to join the Integrated Urgent Crisis Team (IUCT) predominantly working within IUCT Community however the scope of this role will include working across the wider IUCT team which include Admission Avoidance and IUCT Acute. As a member of the Integrated Urgent Crisis Team, you will be expected to work as a senior nurse with a broad range of competencies who is able to adapt to a wide variety of clinical circumstances.

Main duties of the job

To work as part of a multidisciplinary team to complete skilled rapid assessments for individuals referred into the Integrated Urgent Crisis Team either from discharge from the hospital under the discharge to assess pathway or via the community for a rapid visit referred by family, public or other health / social care professionals including North West Ambulance Service and GP's.

Essential

  • oRegistered General Nurse (level 1)
  • oCommunity Nursing / Emergency Nursing / Rehabilitation / Falls or relevant experience in specialist areas
  • oStrong evidence of continued professional development
  • oMentorship / teaching and assessing qualification
  • oAbility to work autonomously and collaboratively, co-ordinating programmes of care
