Job summary

Band 8aAdvanced Clinical PractitionerFull time 37.5 hours per weekPermanent contractThe post holder is a qualified Advanced Care Practitioner, who acting within their professional boundaries, will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of their care. They will demonstrate safe, clinical decision making and expert care for patients within the Same Day Emergency Care unit with the Urgent Care provision. At times of high demand you may be expected to work within other areas of the urgent care division; Acute medical unit or Accident and Emergency department.

In summary, the Advanced Clinical Practitioner role within the Same Day Emergency Care Unit embodies a commitment to excellence, innovation, and continuous improvement within the urgent care provision. By leveraging advanced clinical expertise and a passion for enhancing patient outcomes, the Advanced Clinical Practitioner will contribute to the unit's evolution into a leading area of excellence within the trust.

Main duties of the job

To act as a role model of expert clinical practice.To ensure that documentation is maintained to a high standard. Be a competent user of and participate in the ongoing development of the electronic patient record. Be a competent user of and participate in the ongoing development of the electronic patient record. Ensure that effective discharge planning and health promotion is carried out. To monitor the quality and continuity of patient care and ensure standards are met. Establish and maintain effective communication with nursing, medical and paramedical colleagues to provide optimum patient care. Communicate effectively with patients, relatives, and colleagues. Participate in audit, taking action to address improvements required. Practice in such a way that inspires confidence in all people using the service. Provide advanced levels of practice, knowledge and skills within the Urgent Care Pathway. Develop and implement care pathways for the management of groups of patients within Urgent Care. Promote evidence based practice through the use of the latest research based guidelines and the development of practice based research. Monitor the effectiveness of their own clinical practice through the quality assurance strategies such as the use of audit and peer review.

Is actively involved in initiating and implementing change as directed by the Matron and Lead Consultant.

Perform against quality indicators.

To work in close liaison with other members of the multi-disciplinary team.

To use clinical expertise and skills to accurately assess patients individual needs and to manage clinical priorities appropriately.

Act and behave as an integral part of the team that understands the importance of the achievement of all Emergency Care Standard Indicators not as a set of targets, but as key indicators of good and responsive emergency and urgent care

Participate in the recruitment and orientation of new staff.

Act as a mentor for senior nurses (and other appropriate clinical staff) and provide support for mentees through regular feedback.

Take action to the level of competence gained in managing incidents within the department.

Take action to the level of competence gained in managing and dealing with situations that may result in a disciplinary situation.

Participate in the management and development of the clinical service within the Division.

Participate and or lead in the investigation of Adverse Incidents and Complaints

Work within clinical governance systems and ensure that care is delivered within an appropriate model within the Urgent Care area.

Implement and ensure adherence to all Trust policies.

Develop and implement protocols and or policies within own specialist area.

Effectively contribute to the achievement of Trust targets.

Support the Team Leader and Matron for Urgent Care in effective resource management.
