Follow policies and procedures of Tameside and Glossop integrated Care NHS Trust
The identification of Health needs within a locality corporate caseload
Assessment Organising planning delivering implementation and recording and evaluation of screening programmes and national health promotion guidelines.
Responding to health needs assessments by referring to other professionals (e.g.GPs, Consultants, Speech Therapists), other agencies (e.g. Social Services, housing), and other voluntary agencies.
Identify families with safeguarding issues and support these families by participating in the development of child protection plans as part of a multi-agency forum via case conferences and core group mechanisms.
Participate in all statutory processes in respect of safeguarding including provision of witness statements and court attendance.
Ensure that Tameside safeguarding rules are adhered to.
Undertake programmes of care in partnership with parents, foster parents and other agencies, designed to safeguard children and minimise risk.
Develop and maintain interagency networks.
Facilitate health assessments and plan care for children in the looked after system.
Identify children with disabilities and facilitate support and referrals to maximise developmental potential. Act as a key worker when indicated, ensuring effective, efficient and appropriate care plans.
Provide accessible support to parents on an individual or group basis. To include child development, behaviour management, social & environmental factors that affect health, safety, and public health issues.
Initiate, participate in and evaluate health promotion activities. This may include large group work and activities in public places (e.g. Shopping centres, schools, town halls etc.)
Actively promote community participation and work within a community development framework within a multi-agency forum.
Act as a client advocate for vulnerable members of the community who cannot represent themselves. Facilitate their compliance with treatments and attendance at appointments.
Initiate and participate in research and audit projects.
Demonstrate reflective and evidence-based practice and use this to influence strategy and policy development.
Keep legible, accurate and contemporary records while maintaining client confidentiality.
Collect and record data as required by Tameside and Glossop Integrated Care NHS Trust.
Attending mandatory and Trust training and participating in appraisal.
Accessing clinical and Safeguarding Supervision.
Following Trust policies on flexible working hours where agreed.
Support new or less experienced work colleagues to adapt to the workplace.
Responsibilities for Teaching and Training
Have presentation skills, group facilitation skills and teaching skills in order to deliver courses and facilitate group work on topics, which meet the identified health needs of individuals, groups and communities, and the ability to identify topics most appropriately delivered within a group setting. They will be required to access training as necessary to increase their knowledge base.
Complete assessment documents and any relevant or requested documentation for pre- registered students.
Providing a learning environment for students from many professional disciplines. E.G. trainee GPs, trainee speech therapist, student nurses.
Collaborating with the university to provide a learning environment for student nurses. Responsible for assessment of their practice while in placement.
Participate in training and development activities that are relevant to the job role.
Responsibilities for data and information resources
Able to collect, collate and analyse epidemiological data in order to produce community health profiles and instigate public health responses to local need.
Be computer literate and able to access information via e-mail and web searches and have basic word processing skills.
Research, Development and Audit
Able to work with colleagues to audit the service, contribute to debates in forum. Support to Initiate, design and evaluate procedures that govern health-visiting/school nurse practice. Ensure that current practice operates within the Clinical Governance Framework.

Undertaking own data collection for audit purposes, community reports and to substantiate bids for funding. Act on the results/outcomes.
Analysing data collected and using information to influence change in own practice and that of others. This may extend outside own professional sphere to the community.
Ensuring a high quality of service provision by participating in research, audit and benchmarking activities as required by the Tameside and GlossopIntegrated Care NHS Trust in line with the Clinical Governance Framework.
Physical Skills and Effort
Required to use car on a daily basis, throughout the day.
Required to transport appropriate equipment to home visits, schools and other clinical venues.
Required to set up specialised clinical areas including moving furniture.
The work requires kneeling, bending and twisting during health assessments in the home environment, school or clinic setting.
Concentration is required during periods of every working day in client interaction. The post holder is required to have the ability to assess the situation, be supportive and give information and advice at a level assessed to be appropriate to each client. A risk assessment is also carried out. The post holder negotiates a strategy and a package of care with the clients and will evaluate its effectiveness at subsequent contacts.
Regular report writing for court reports, child protection conference reports, child development assessments and reports, reports to hospital consultants, require a high level of knowledge, concentration and skill.
The post holder is required to maintain an awareness of current legislation e.g. Children Act, Nurses and Midwives Act, Data Protection Act, National Service Framework and work within these parameters.
The post holder is required to have the ability to prioritise within a work pattern which is unpredictable, amid constant interruptions and a necessity to respond to crisis e.g. domestic violence, child protection issues, and mental health issues, social issues (including housing, financial crises and substance misuse). Work is regularly interrupted by social services requiring information for child protection issues, by hospital consultants requiring medical history, by GPs requiring support and by clients requiring support and information.
Daily involvement to address a variety of emotional issues such as bereavement, domestic violence, suicide and self-harm, drug and alcohol misuse, child protection, homelessness and mental illnesses.
Required to document and evaluate highly distressing events and reproduce information in records and reports for child protection conferences, core groups and court statements. This process may continue over lengthy periods, sometimes indefinitely.
Provide emotional support to other staff members through clinical supervision and/or team management in times of stress, change or uncertainty.
Post holder can be the first person to discover serious incidents such as serious non-accidental injury, neglect, and domestic violence. They will provide a service to distressed and/or angry clients who may be the perpetrators as well as victims of domestic abuse, child abuse, and substance misuse
The post holder is required to work alone, following a lone worker policy. The work environment involves working in clients homes and community buildings.
