Senior Digital Health Assessor Band 6 -37.5 hours - Full Time, Permanent
We are looking for Individual Nurses and /or Paramedics with a sound clinical knowledge and experience of an acute setting/background working with Adults you will have excellent assessment skills demonstrating a logical decision making process that ensures patient safety.
This post will require you to utilise your clinical assessment and management skills to enhance the experience of all appropriate patients under our care to access and use the facilities / services available via Digital Consultation & telephone triage assessments.
This post will enable early supported discharge, prevent inappropriate re-admission to hospital, share nursing knowledge and skills and support the technological advances which are being developed at Tameside and Glossop Integrated Care NHS Foundation Trust.
The Senior Clinical Assessor will support and participate in the development of integrated pathways and care packages which enable patients to be safely cared for outside of the acute hospital setting and support palliative / end of life patients through a designated telephone service.

Main duties of the job
The Digital Health Service is currently being developed to support Tameside and Glossop's new Model of Care. This post provides the opportunity to lead the way in digital health in Tameside and Glossop be part of a new and emerging team.
This post will require you to utilise your nursing and clinical skill to enhance the experience of all appropriate patients under our care to access and use the facilities/services available via Digital Consultation.
This post will enable Adults & Children to be cared for safely by remotely supporting established triage assessments & care packages at home or in their place of residence.

Section 1 - Main Responsibilities
Professional/Clinical
- Provide the delivery of evidence based nursing care by providing expert clinical assessment, care planning, care delivery, care evaluation and clinical advice in accordance with NMC Code of Professional Conduct.
- Ensure all records relating to patient care are accurate, clear, concise, legible and in accordance with the NMC Standards for Records and Record Keeping.
- Maintain a high standard of clinical care for all patients utilising Digital Health in accordance with Trust guidelines and protocols.
- Participate in the provision of early intervention with appropriate assessment, observation and treatment for seriously ill patients within the Digital Hub and within the trust under the supervision and support.
- Support the development of care plans for Intermediate care and complex care Patients, referring to other services as required.
- Work alongside the hospital at night service.
- Participates in practice development programmes and assists with the audit and evaluation of clinical standards.
- Participate in the in-service training, preceptorship and supervision of junior staff, post-basic course nurses and students, with a commitment to self-development.
- Develop good Trust wide working relationships and communicate effectively with nursing colleagues and members of the multi-disciplinary team.

Clinical Expertise

- Take responsibility for the assessment of acutely ill patients who contact the Digital Care Hub service via telephone and video.
- Competently assess and triage acutely ill patients intermediate care patients and complex care patients using the digital care service, including the assessment, planning and evaluation of programmes of care.
