Band 6Full time 37.5 hoursPermanent ContractWe are looking for a motivated and enthusiastic Band 6 Dietitian to join our friendly team of Community Dietitians.

The post will suit a dietitian who thrives on variety and wishes to experience patient care in a range of different settings (clinics/ home visits/care homes and intermediate care)
You will develop skills in a range of clinical areas including nutrition support, gastroenterology, liver and pancreatic conditions, diabetes and other long-term conditions.
You will enjoy managing patients holistically in their community setting, liaising with acute dietitians, tertiary centers and other health professionals as needed
You will be an excellent communicator
Quality improvement initiatives are welcomed in the department. You would be encouraged and supported in considering improved and more effective ways of working.
If successful you will join a friendly team with 9 community dietitians, a dietetic assistant practitioner and clerical support based within easy access of Manchester and the peak district.
We have strong links with the Acute Dietitians

Main duties of the job
The post holder will provide Dietetic treatment for adults based in community settings.
The service will be provided in a range of locations including clinics, patients own homes, care homes and will provide part of Dietetic Care the intermediate care unit
You will be interested in a wide range of conditions and maintaining broad dietetic skills and will enjoy working both with patients and other health professionals.
You will be involved in the training of student dietitians during their placements.

- To be responsible for a caseload of patients in a range of locations including intermediate care.
- To carry out detailed individual nutritional assessments, treatment plans and advice for a range of conditions including diabetes, coeliac disease, GI problems, intolerances and oral nutritional support, liver and pancreatic disorders
- To interpret biochemistry appropriately and request as needed for re-feeding syndrome
- To manage tube fed patients when on the intermediate care unit
- To work holistically taking account of medical and social information from a range of sources.
- To arrange joint visits where needed e.g. with SALT
- To take responsibility for ensuring that patients and carers have consented prior to dietetic intervention, in line with professional guidelines.
- To consider the practical implications of advice and tailor information to the individual circumstances of the patients, families and staff involved.
- To liaise with relevant health professions regarding care provision to ensure an integrated approach to care, ensuring all relevant agencies have the necessary information to support the client in meeting their needs.
- To ensure that an agreed standard of service and patient care is delivered in line with professional standards.
- To facilitate referrals to appropriate agencies and to liaise with appropriate health care professionals if concerns are identified
