Job summary

Cancer Patient Pathway Navigator Band 4 37.5 hours per week Fixed Term- 18 months

The post holder will act as the patient's navigator tracking their timed pathway against the national guidance.

The post holder is responsible for supporting patients and their families through the relevant cancer pathway across secondary, community and primary care as required by the patient. They will be responsible for coordinating care, education and support for people with complex care needs and providing a point of access to patients and carers. This post is part of a new patient pathway that aims to deliver a diagnosis within 28 days from referral.

The development of the role will be led by the needs of the patients. The post holder must be adaptable and willing to work flexibly as the service progresses.

They will be responsible for entering accurate data onto the Trust's Electronic Patient Record and Somerset Cancer Register database

Main duties of the job

On the 9th November 2021 the Government announced new regulations which we anticipate will come into force on 1 April 2022, requiring people who conduct Care Quality Commission (CQC) regulated activity in public (NHS) or independent sector health and social care settings in England, to have received two doses of a Medicines and Healthcare products Regulatory Agency (MHRA) approved COVID-19 vaccine unless exempt.

Please be aware that provided these regulations do come into force as currently planned, this role will be affected and as such, successful candidate(s) will be required to show proof that they have been fully vaccinated against COVID-19 as part of the pre-employment check process. Job responsibilities

MAIN RESPONSIBILITIES

General

Coordinate care by providing a single point of access, including rapid re-entry into the system for those people identified as having urgent needs

Provide advice and refer on or sign-post to other sources of support

Coordinate diagnostic and monitoring through appointments and investigations

Coordinate face to face appointments for those with complex results.

Discuss with patients issues relevant to their health and social care and provide support where possible or escalating relevant clinical teams if necessary.

Maintain accurate patient records on both the Trust Electronic Patient Record and the Somerset Cancer Register to reflect the care given.

Contribute towards the maintenance of local and Trust wide standard of care Inform the clinical team/person in charge of:-any changes in the patients physical condition or situation any significant factors relevant to the patient.

The jobholder will be expected to carry out other duties assigned by the team, which are appropriate to the pay band and the training programme.

Communication

Ensure a smooth transition of care along the clinical pathway ensuring clear communication between the patient, clinical nurse specialist, consultants and both internal and external clinical support teams.

Maintain confidentiality of patient information

Contribute to the effective communication process between patients, carers, family, friends and staff colleagues

Support people to access appropriate information and support by sign posting to a range of support services and take an approach which helps people to self-manage where appropriate

Maintain clear, timely communication with the relevant Cancer MDT Coordinator/ Tracker and to attend and participate in MDT meetings when required

Contribute ideas to the multi professional team

Support the delivery of patient and carer training and education

Encourage and support healthy lifestyle choices.

Education and Training

The post holder will require to be fully up-to-date with regard to waiting list targets/timescales/cancer targets.

Ensure all mandatory training is maintained in accordance of Trusts guidelines and policies

In house training encouraged and external training supported

Continuous involvement and implementation of procedure developments in the Units to assist in improving the patients journey.

To keep up to date with developments within the relevant Cancer service, Trust and the wider Greater Manchester Cancer agenda.
