This is a new and exciting role for the organisation and applications are welcome from nurse specialist with expert knowledge and skills in the the management of patients with parental and enteral feeding regimes. The post holder will form part of expert MDT.

Main duties of the job
Support for enteral and parenteral nutrition. The post holder will be an expert clinician who will provide training and demonstrate clinical excellence in the areas of enteral and parenteral nutrition to nurses, doctors and other health care professionals. They will guide, supervise & advise to ensure the highest standard of care for our patients. To engage in audit and research activity to measure effectiveness of care & to provide assurance to continue reduction in nutrition related incidents
To prevent avoidable admissions related to enteral nutrition. To be a conduit between the acute and community to develop clinical pathways to decrease and prevent avoidable admissions e.g. tube related problems.

Responsibilities of the role: To provide evidence based highly specialist nursing advice and input for patients with complex nutritional needs and those requiring artificial feeding. To work with the infection control team to reduce line infection rate by delivering high quality line care training package and auditing practice. To support patients requiring complex nutritional treatments; to navigate their pathway of care from referral through treatment to discharge and follow up, where appropriate for the following conditions: suffering from intestinal failure on Parenteral Nutrition (PN) requiring Enteral nutritional support within the hospital support the discharge process for patients receiving Enteral Nutrition Working alongside the existing clinical education structures to ensure an adequate skill mix of Healthcare Professionals competent in: passing nasogastric tubes caring for the patient on Enteral Nutrition caring for the patient on Parenteral Nutrition (linking in with Infection control to ensure safe practice with central venous access) Support the hospital in meeting the following standards and recommendations: NICE CG32 Nutrition Support in Adults NCEPOD- A mixed bag 2010 Promote audit and research Report to governance on issues relating to nutrition. Attend cross divisional groups, forums and boards to highlight nutritional issues.
The role demands the commitment to provide nutritional clinical support to patients, their families/carers and health professionals involved in the patient journey.Communications Maintain confidentiality of staff and patients. Maintain accurate records. Attend regular meetings with the divisional matron/Head of Nursing. Ensure patients receive appropriate information and support in relation to their cancer and treatment. Facilitate open and effective communication within the specialist team and external organisations, both verbal and written, often at a senior level. Ensure patient pathways are supported by appropriate documentation e.g. key worker handover. Develop links with clinical networks and other specialist services to facilitate the sharing of good practice as required. Lead and manage change within a complex environment in response to emerging knowledge and national cancer initiatives, using persuasion and influencing skills. Work collaboratively with other professionals, MDTs, SMDTs, and other organisations to ensure that patient and carer needs are met, ensuring the involvement of the multi professional team as appropriate. Work collaboratively as a member of the MDT, communicating and leading on issues relating to specialist nursing care and management of the patient pathway, ensuring patients needs are effectively represented and addressed. Ensure timely and effective communication with patients, staff, relatives and carers in relation to care issues, often managing complex and sensitive information. Proactively manage patient, relative and staff concerns, liaising with the relevant departments as necessary. Seek support and advice on matters outside experience and authority.Clinical responsibilities As part of the Nutrition MDT supporting the assessment of inpatients referred for complex enteral / parenteral nutrition, providing guidance to the clinical team. Attend weekly nutritional support team meetings where applicable Be called upon to assess patients coming into A&E/AMU/SAU & SDEC with tube related problems Be called upon to insert nasogastric tubes when ward staff are unsuccessful or are having difficulties. ENT provide support to patients who have had a PEG placed & ensure follow up on the ENT ward post PEG insertion to avoid complications.Provide support to the ward nurses in post PEG care and with any tube related problems that occur with patients with existing tubes. Assess patients who require nasal bridles to secure nasogastric tubes and insert nasal bridles as appropriate / required. Place peripheral feeding cannulas & place PICC lines, gaining consent as per trust guidelines.Responsibility for Patient Care Establish contact with patients/ carers from point of initial referral/identification to providehighly specialist nursing advice and practical /psychosocial support. Communicate with patients in order that they are empowered to make informed choices about their health and health care. Act as advocate for individual patients and client group. Works alone or in collaboration with medical staff to provide expert knowledge in the assessment, care planning, education and advise for people with complex nutritional needs Assess, plan, implement and evaluate programmes of care for patients to meet identified nutritional needs, in partnership with patients/carers and health professional colleagues. Receive referrals from other health professionals within the health service. Liaise with primary care and other agencies to provide continuity of holistic patient centred care. Work closely with the home enteral feeding dietitians to provide appropriate support and prevent hospital admissions.Planning and organising As part of the Nutrition Team assess in-patients referred to Endoscopy for a PEG, provide direction to the managing clinical team and work with Endoscopy to prioritize PEG insertions according to scheduled lists. Develop a nurse led PEG services in conjunction with the MDT In conjunction with MDT monitor the appropriateness of parenteral nutrition. To be involved in decisions regarding purchase of enteraland parenteral associated feeding equipment to ensure the best priced & best evaluated equipment is used. Organise own personal development with regard to mandatory training and revalidationrequirements. Planning own annual leave considering and planning around the needs of the service. Arranging Integrated Trust multi-site activity considering the travelling time.
