Job summary
This is an exciting opportunity for an experienced dietician to join the newly formed Proactive Care Team as part of Bolton's response to the NHS 10 year plan to move care from hospital to home.
The service operates Monday - Friday and offers flexi-time working arrangements, recognising the importance of balancing the needs of work with family commitments. The post is a 18.5 hours/week post.

Main duties of the job
To work as an autonomous practitioner within the Proactive Care Team, supported by a multi-disciplinary team, including primary care.
To perform therapeutic assessment of patients in their home,with diverse presentations and complex physical and psychological conditions, to provide a diagnosis and develop and deliver individualised treatment programmes.
To manage a defined caseload, using evidence based, client centred principles to assess, plan, implement and evaluate interventions.
To provide clinical education, advice and support for junior therapy staff and support staff.
To providehighly specialist knowledge, skills and advice to clients, and be a source of expertise for therapy staff, other members of the MDT and other agencies.
To supervise, educate and assess the performance of students.
To contribute to service research and audit activities.
To propose any service changes within own professional speciality area.
