Job summary
This post offers the opportunity for registered Healthcare Professionals to complete the 2 years MSc Advanced Practice through the Bolton University in the September 2025 cohort. Your role will enable you to study with the university and have practical on the job training at Bolton NHS Foundation Trust with the support of an established Acute Frailty ACP Team and a large team of Geriatricians through the Apprenticeship scheme.
The Trainee ACP will be working towards becoming an autonomous, accountable practitioner who will work on their own initiative and in collaboration with medical, nursing, and allied health professional colleagues as required.
Until signed off as competent, the Trainee ACP will work under the supervision of a qualified clinician whilst developing the skills that enable them to become an autonomous practitioner.
The role will be structured around the four core functions of advanced practice:
- Expert clinical practice (with a minimum clinical focus of 80%)
- Professional leadership and consultancy
- Education, training and development of self and others
- Practice and service development, research, and evaluation
We are welcoming of applications from all registered health care professionals (nursing, pharmacy, paramedics and occupational therapy) who have the ability to complete the non medical prescribing course.
Main duties of the job
The trainee Advanced Clinical Practitioner (ACP) will assess, diagnose, treat and make the decision to admit or discharge frailty patients safely ensuring appropriate advice, health education and referral to appropriate services or other members of the MDT under the supervision of the Acute Frail & Complex Care team.
The Frailty Team in-reach into the Emergency Department (ED), AAU1 and AAU2 which are short stay assessment areas, assessment units D1 and D2 with a large focus on the Older Person's Assessment unit located on B3. They work alongside our Geriatrician team and ED and Acute Medicine teams to ensure the highest standard of care and treatment for our frail population.
The Trainee ACP post holds the expectation that following completion of the MSc , you will be offered a qualified ACP post (subject to completion of the competencies and academic requirements of the apprenticeship).
The post is blended with 2 days of study leave and 3 days on site with the Frailty Team 7am-5.30pm, this includes some weekend working.
About us
The Acute Frailty team are an established team with a wealth of experience and provide the front door (ED, acute areas and the Older Person's Assessment Unit) with frailty knowledge and clinical leadership.
We hold a Frailty Steering Group every 2 months to discuss areas of development, training opportunities, performance data and it enables an opportunity for wider working with other divisions and community partners.
The Frailty Team consistently achieve the CQUIN05 target every month whereby patients with grades of frailty Clinical Frailty Score (CFS) 6 orabove should be assessed for frailty associated syndromes via acomprehensive geriatric assessment.
