Job summary

Are you a Band 5 Physiotherapist looking to develop your skills at a Band 6 level? Are you a Band 6 Physiotherapist who would like to develop your clinical skills in a different area?

If so, we have a brilliant opportunity to come and join us here in the District Therapy Team at Bolton Foundation Trust.

We are seeking to appoint a dynamic, confident, innovative and flexible individual with excellent communication and organisational skills. The successful individual will need to engage closely with acute staff, community colleagues and social care staff to coordinate and provide high quality care in the community to avoid hospital admission and support hospital discharges.

Service is over 7 days 8am-6pm servicePrevious applicants need not apply.

Main duties of the job

In order to successfully undertake this post you must be an experienced Physiotherapist looking to develop in this clinical area.

You will need some knowledge of community/intermediate care services.

You will be an effective team player, inspirational, flexible, adaptable, enthusiastic and fully committed to the development of patient focused, clinically effective and efficient services.

About us

The role will be part of a multi-disciplinary team consisting of Physiotherapists, Occupational Therapists, Therapy Instructors, Pharmacy, Dieticians and Speech and Language therapists. You will be a key player in providing assessment, personalised care planning and management of care to patients who may require intensive home support as an alternative to hospital care. The role will include participating in effective multi-disciplinary team meetings and close working with the Local Authority.
