Job summary

The post holder will work closely with the specialty clinical leads and senior management team within diabetes to support the effective clinical strategy and operational management of the specialty service, ensuring a high quality patient care within the resources available.

The post holder will be a, highly visible professional who is approachable to patients and carers and staff within the Diabetes team. To work as an autonomous practitioner within agreed protocols and to deliver a specialist service to patients with Diabetes and their families in conjunction with the lead clinicians and specialty team.

The post holder will contribute to achieving the objectives of the business unit and corporate objectives and work within the clinical governance framework.

In partnership with other members of the multi-disciplinary team the post holder will provide expert nursing care, tailored advice, education and support to Diabetic patients and their relatives/ carers.

They will lead the development and delivery of a programme of practice and professional development for staff within the specialty to ensure a high quality of care is delivered.

The post holder will be required to manage the team of specialist nurses and support staff within the Diabetes team on a daily basis providing professional leadership and supervision.

Main duties of the job

Perform specialist assessment of patient's clinical needs, plan, implement and evaluate care delivery using advanced clinical knowledge, clinical examination and clinical skills.

To be clinically credible in the area of Diabetes and other aspects of these diseases.

Contribute towards a multi-professional approach in the management of patients, especially in relation to ongoing care needs and discharge arrangements.

Support patients and their families and ensure that they receive the required information to enable them to self-manage, lead or participate in their care delivery.

Review patient's medication regime, ensuring that patients receive appropriate medication in effective doses, and monitor patient's clinical status and blood chemistry following medication changes.

To ensure that nursing is underpinned by evidence-based practice where applicable and encourage the application of nursing research into daily practice.

Ensure seamless transition of care from secondary to primary care.

Provide accurate discharge information for GPs and community teams to facilitate effective follow up care and management for patients who access the service.

Co-ordinate and implement a nurse led outpatient clinic for patient requiring ongoing care assessment and counselling services.
