Job summary
An exciting opportunity has arisen for an aspiring, self-motivated individual to join Bolton Foundation Trust's Integrated Community Services Divisional as a Transformation Programme Project Manager .
You will work closely with the Reactive Care Lead and services within their portfolio on a number of key agenda's such as ageing well, urgent care and discharge and flow. You will join a team from a number of disciplines and backgrounds and work collaboratively with the Reactive Care leadership team to deliver key priorities.
Main duties of the job
The post holder will work alongside and support the Reactive Care lead and the Head of Neighbourhood transformation to ensure that the current system wide strategic transformation programmes of work (community) is managed using defined project management principles. The post holder will assist and support the wider team in the delivery of the community priorities and operational planning, to transform and build community services capacity to deliver care out of hospital and improve hospital discharge:
- Admissions avoidance, developing and embedding community pathways to avoid admissions
- Discharge improvement initiatives improving patients flow
- Digital transformation, embedding innovations to enable and support admission avoidance, discharge, and other clinical networks.
- Understanding system-wide community service resource (demand & capacity) to support planning.

- Knowledge of project management processes and delivery
- Experience in a similar position within the public sector, NHS and local authority
