Job summary

The future of Health Care is changing and the Admission Avoidance Team is crucial to delivering the vision, therefore we are expanding our service to enable us to deliver innovations such as the Virtual Ward and Hospital at Home model of care. We are a multidisciplinary team of expert clinicians who support service users in crisis to remain at home. We are looking for an inspirational Occupational Therapist who would like to further develop their clinical and leadership skills within a friendly and dynamic team.

You may be wanting to progress from a Band 5 role or looking to diversify your clinical experience within our fast paced and exciting team. You must be passionate about motivating and engaging staff to ensure the delivery of effective and efficient high quality care and be comfortable with change.

Main duties of the job

The successful candidate will work within a team consisting of Advanced Clinical Practitioners, Nurses, Physiotherapists, Occupational Therapists, Support Workers and Nurse Associate who operate 7 days a week between 08:00 and 22:00. As a Band 6 Occupational Therapist you will be expected to support the team to deliver a high standard of care through the demonstration of excellent clinical and leadership skills. You will be an effective team player who possesses high level problem solving skills and who can readily adapt to the changing demands of the service is essential.

As part of the role you will work very closely with the other teams within the Intermediate Tier to achieve our goal of preventing avoidable hospital admissions and facilitating discharge from hospital. Intermediate Care services are continually developing and this is an exciting opportunity to be at the forefront of developing an essential service that is pivotal in preventing hospital admissions.

Qualifications

Essential

  • Degree or equivalent in Occupational Therapy
  • Current professional registration with HPC as Occupational Therapist
  • Evidence of CPD in related field of clinical expertise
  • Current accredited field work educator.

Desirable

  • Post graduate qualification in specialist field

Knowledge

Essential

  • Understanding of the quality and governance framework and implications for practice including audit and research governance.
  • Understanding of the Health & Social Care Integration agenda
  • depth understanding of current NHS/government policy/Public Health/NICE guidelines
  • Change management skills
  • Excellent time management skills

Experience

Essential

  • Treatment of a broad range of conditions relating to clinical speciality
