Job summary

The MDT Coordinators co-ordinate the MDTs across the Borough; they are the one point of referral, they ensure the invites go out set up and minute the meetings updating the GMCR so information is accessible for GPs, NWAS and hospital staff. They take and triage calls for the integrated district teams ensuring people and referrers get to the right professional.

Main duties of the job

The MDT co-ordinators fulfil a critical role in integration. One of three better care funded posts in each district (serving 3 primary care neighbourhoods); they co-ordinate the MDTs across the Borough; they are the one point of referral they ensure the invites go out set up and minute the meetings updating the GMCR so information is accessible for GPs, NWAS and hospital staff. They have access to all systems (LA GMMH and Allscripts) so undertake background checks on all new referrals to the neighbourhood / district staff. They take and triage calls for the integrated district teams ensuring people and referrers get to the right professional. The vacancy is in the South where the MDTs are well utilised with good system engagement particularly from primary care. South district is the area with the highest rates of deprivation and health inequalities; integrated working and MDTs are a critical way of addressing this. The proactive care model soon to be launched will involve using multiple data sources to identify people who are at risk of deterioration who will benefit from MDT support to prevent crisis. The numbers of MDTs are likely to increase significantly. The MDT co-ordinators also set up new records, file and archive and can in the future be trained in civica to support scheduling and capacity and demand management.
