Main duties of the job

  • Provide specialist advice and support to patients, their families/carers and other professionals with regard to chronic heart failure.
  • Deliver and support co-ordination of holistic heart failure care through delivery of an MDT in-reach service for in-patients through to discharge.
  • Manage patient care and adjust treatments within local and national guidelines and evidence base.
  • Independently manage a caseload of patients with heart failure and provide outpatient care to patients within a hospital, community clinic or within the patient's own home.
  • Be actively involved in the development of an integrated chronic heart failure pathway within the hospital and the community.
  • Lead and develop local action/management plans to meet the supportive and palliative care needs of patients with advanced heart failure.
  • Work closely with the multi-disciplinary team (MDT) to deliver effective management of patients with heart failure.
  • Exercise high levels of judgement, discretion and decision making in clinical care.
  • Be responsible for education and training of the MDT in the management of heart failure.
  • Be responsible for data collection and completion in order to meet local and national audit requirements for patients with heart failure.
  • Act in accordance with the NMC Code of Professional Conduct.

About us

The heart failure service at Bolton NHS Foundation Trust is honored to be able to offer a fully integrated service for our heart failure patients operating from a shared team hub. We are an ambitious and progressive team and we are excited to be expanding both our hospital and community service to facilitate early in patient review and review post discharge from hospital, with the aim to reduce admissions and improve patient care. This role is pivotal for the assessment of patients and families referred to our service and integrated working provides a seamless pathway of care linking the hospital and the community.

We are looking for someone enthusiastic, caring, open minded and flexible who can bring a dynamic approach to the management of heart failure patients to join our established team.

Applicants will have a minimum of 5 years post registration experience within the Cardiology speciality, and experience as a band 7 Heart Failure Specialist nurse. However, we also welcome nurses who hold experience within the speciality of cardiology who are interested in developing their knowledge and skills within a specialist area. This post is also being offered as a band 6 training opportunity with progression to band 7 upon completion of training modules and competencies. The appropriate support and training will be provided.
