Advanced Clinical Practitioner - Band 8a

We have an opportunity for an Advanced Clinical Practitioner to join our established Urgent Response Team (URT).

The URT accepts referrals from GP's, NWAS, ED clinicians and other health care professionals. The service vision is to enable patients to remain in the own homes and prevent unnecessary ED attendances or non-elective admissions by providing a timely response from referral to assessment to support a community wrap around service for up to two weeks for adults in their own home.

We run lots of pathways that enable patients to remain I there own homes .

Operational Hours are 08:00-22:00, 7 days week so will involve some out of hours late evening and weekend working.

The team is multi-disciplinary with Advanced Clinical Practitioners, occupational and Physio Therapist, Nurses, Support Workers and Social Workers. We are also supported by Community and Hospital based Consultants.

You will be able to evidence advanced communication skills, work well in teams, be adaptable and well organised and work well under pressure, you must also hold a full and valid driver's licence and have access to your own car.

Main duties of the job

Our Team provides a 7-day urgent and timely response to referrals from the ambulance service and other health care professionals. Operating hours are currently 0800-2200.

We are a clinically led multi-disciplinary team of advanced clinical practitioners, occupational and physiotherapists, nurses, support workers, pharmacists and social workers. We are supported by community and hospital based medical consultants.

Our teams aim is to support adults with a health or social care crisis to remain in their own home, seeking innovative alternatives to hospitalisation wherever possible.

The role involves attending patients in the community with undifferentiated acute illness and the assessment, examination, diagnosis and planning to enable a holistic approach to their care.

You must be able to travel around various community locations using your own vehicle with business insurance to deliver care.
