Your Role:As a key member of our team, you'll provide high-quality, proactive care tailored to the needs of your designated neighbourhood population. Utilizing your specialist skills in assessment, planning, and implementation, you'll deliver person-centred care that makes a positive impact.

Main duties of the job

Key Responsibilities:

  • Collaborate with Primary and Secondary care clinicians, hospital teams, voluntary organisations, and other professionals
  • Ensure the right care is delivered in the right place, by the right professionals
  • Share caseloads within the multidisciplinary team to maximise efficiency and patient outcomes
  • Demonstrate leadership, teamwork, delegation, and excellent care planning
  • Manage medication, health, and case coordination for individuals with complex needs

Essential

  • Registered Nurse with current NMC registration
  • Nursing degree or equivalent
  • Clinical skills or a commitment to undertake within 12 months of appointment
  • Evidence of recent CPD
  • Commitment to innovative working and new developments
  • Developed essential empathetic communication skills
  • Ability to develop long lasting sustainable professional relationships
  • A willingness to undertake the V300 non-medical prescribing course within 18 months of appointment
  • IT skills

Desirable

  • V150 non-medical prescribing qualification
  • V300 non-medical prescribing
  • Mentorship qualification or equivalent
  • Community experience
  • Evidence of working with patients with long term conditions
