Your Role:As a key member of our team, you'll provide high-quality, proactive care tailored to the needs of your designated neighbourhood population. Utilizing your specialist skills in assessment, planning, and implementation, you'll deliver person-centred care that makes a positive impact.
Main duties of the job
Key Responsibilities:
- Collaborate with Primary and Secondary care clinicians, hospital teams, voluntary organisations, and other professionals
- Ensure the right care is delivered in the right place, by the right professionals
- Share caseloads within the multidisciplinary team to maximise efficiency and patient outcomes
- Demonstrate leadership, teamwork, delegation, and excellent care planning
- Manage medication, health, and case coordination for individuals with complex needs

- Nursing degree or equivalent
- Clinical skills or a commitment to undertake within 12 months of appointment
- Evidence of recent CPD

- A willingness to undertake the V300 non-medical prescribing course within 18 months of appointment
- IT skills
