Job summary

We are pleased to offer an exciting opportunity for a Band 4 professional to join our dedicated diabetes team, working alongside consultants, specialist nurses, and dietitians.

We're looking for a highly motivated individual with a passion for developing their skills, particularly in patient support and education around self-management of diabetes.

In this role, you will play a key part in delivering education and advice to patients, families, carers, and other healthcare professionals.

The ideal candidate will have a strong interest in diabetes care, a proactive approach to learning, and a commitment to delivering high-quality education and support to help improve patient outcomes.

Main duties of the job

  • Providing one to one education and structured group education to patients with diabetes. Coordinating group education sessions
  • Supporting patients with the initiation of diabetes technology such as continuous glucose monitoring systems.
  • Supporting consultants in clinics
  • Visiting patients at home to assess and support them with their diabetes

  • Previous experience working within the health care setting as a health care / assistant practitioner
  • Experience of undertaking clinical observations and procedures, interpreting the results, and escalating appropriately
  • Experience of venepuncture

  • Experience of working with the public

Desirable

  • Evidence of continuous professional development (CPD)
  • Evidence of foot surveillance training or willingness to undertake
