Job summary
We are seeking a dynamic, experienced and highly motivated Pharmacy Technician to join our established pharmacy team at the Bevan Unit - a purpose built 60-bedded Intermediate Care Unit, located near the hospital site, which helps patients regain their independence before safely returning home after a hospital stay.
The successful candidate will work with an integrated multidisciplinary team of healthcare professionals and participate in a range of medicines optimisation activities to support patients get the best outcomes from their medicines.
The post requires you to be enthusiastic, resilient, adaptable and able to plan/manage your time to meet the needs and goals of the service.

Main duties of the job
This is a patient-facing role. The post-holder will work alongside a team of highly skilled Pharmacists, Pharmacy Technicians and Medicines Management Assistants, to deliver high-quality medicines optimisation and support within Intermediate Care.
Main duties will include medication history taking, identifying and resolving medicines-related issues, assessing medicine adherence and counselling patients on their medicines, dispensing medicines for in-patient use and accuracy checking discharge summaries for Intermediate Care. The post-holder will attend MDT meetings, providing advice to health and social care professionals. The post-holder will also work across the interface, liaising with hospital and GP teams to support the safe transfer of care and improve medicine safety.
As this is an ever-evolving role, the post-holder will contribute to new service developments and projects, to facilitate improvement and changes.
There is currently no weekend/late night commitment with this role.
