Are you looking to expand your clinical skills?
We are seeking a self-motivated, enthusiastic pharmacist to join our experienced clinical pharmacy team within Community Services at Salford.
Working in a variety of community settings including Intermediate Care, Care Homes, and Home First, you will support patients and the wider team to ensure that people under our care get the most from their medicines. Based in the community, we work closely with the Acute Pharmacy Department. This includes access to training and career-development opportunities.
You must have excellent practical clinical knowledge and able to communicate effectively with fellow professionals, support staff and patients. You must be confident working independently with support provided by our senior team.

Main duties of the job
The clinical pharmacy role within Community Services can pro-actively help patients remain well and out of hospital, reducing demands on the acute Trust. The role supports patients to get the best outcomes from their medicines, working as part of multidisciplinary teams.
Key responsibilities for the post:
- Provide a high-quality clinical pharmacy service within the community setting.
- Participating in MDT and Consultant Ward rounds.
- Identify and resolve medicines-related issues on transfer between care settings
- Ensuring safe practice and development of guidelines and procedures.
- Education and training of staff on safe and appropriate use and prescribing of medicines.
- Support the delivery of other community medicines optimisations and medicine safety workstreams when required.
There is currently no on-call or weekend/late night commitment with this role.
A valid drivers' licence and access to a car for work purposes is essential.
