We are currently looking to recruit qualified Health Visitors to our developing SEND Health Visiting Team. The team will provide an enhanced pathway of support for children with additional needs. The successful candidates will have strong team working skills and a commitment to delivering high quality, child-centred care, to join our developing, enthusiastic and dynamic Health Visiting teams in Bury.

To have continuous responsibility for the clients on the caseload requiring care and targeted support at universal partnership plus (UPP) and safeguarding thresholds. Health Visitors will have a responsibility for participating and influencing decision making at professional meetings to ensure the complex needs of the children on this caseload are supported.

You will need to be a registered nurse or midwife and Health Visitor.

The Health Visitor will participate in the identification and safeguarding of vulnerable children and their families in partnership with other agencies.

As a member of the Integrated Children's services in Bury, the post holder isexpected to contribute to the expectations of the Borough's Children and Young Peoples Partnership Strategy and the Early Help Strategy.

Main duties of the job

The Health Visitor will be responsible and accountable for the operational and strategic management of a defined caseload of children with complex medical conditions and SEND. The post-holder will be responsible for the delivery of high quality programmes of care to support infant's, children and their families using a multi-disciplinary approach.

To act as a resource and provide consultation for the wider health visiting service to share expert knowledge and skills to support children with additional needs. Support at home visits, in early years educational settings and attend team around the family (TAF) meetings to be offered and advice to be given.

To support the health visiting service with current developments in transition processes and contribute to supportive transitions for children between services and into school health and education.

Utilise specialist public health knowledge to provide high quality assessment and care planning of parents and caregivers physical health and emotional well-being to ensure that they remain as healthy as possible.

The Health Visitor will participate in the continued development of the SEND health visiting team and wider community service.

The Health Visitor will manage the development,implementation and compliance of policies, procedures, audit and benchmarks, including appraisal, personal development plans and clinical supervision.
