Join our Children's Community Nursing Team (CCNT) in Oldham on an 8 -month fixed-term contract to cover band 5 back fill . Main duties of the job

Join us in delivering exceptional, evidence-based care to children in the comfort of their own homes and community settings! As part of our dedicated team, you will partner with parents and caregivers to provide tailored support for every child's unique needs.

In this dynamic role, you'll monitor and recognize acute illness exacerbations, initiate critical nursing interventions, and collaborate closely with senior clinicians for optimal care. Your expertise will help shape individualized care programs through ongoing assessment and evaluation, ensuring effective communication with multidisciplinary teams.

You'll empower children on their journey to self-care, guiding them toward healthier lifestyles while educating parents and caregivers about health promotion and specific clinical needs.

Additionally, you'll connect community services with children's inpatient units, facilitating smooth transitions and safe discharges. Your responsibilities will extend to community clinics, outpatient appointments, discharge planning meetings, and safeguarding discussions, ensuring vital information flows seamlessly among all parties.
