Job summary

An opportunity has arisen for a motivated, enthusiastic and innovative individual to join our COPD team.

You will have a keen interest in the field of lung cancer care with experience working in this area at band 6 or above and will already be working within an early supported discharge team. You will provide valuable support to patients who have been are under the care of the lung cancer clinic and act as a role model to staff working outside of the expert team.

You must possess excellent interpersonal skills as well as the drive, vision and initiative to motivate others and meet the demands of the service. You will be experienced at maintaining patient records and databases and have knowledge of the current cancer pathways.

This post is a permanent post and to ensure that the requirements of the service are met, is full time.

Main duties of the job

  • You will be expected to work collaboratively with patients and professional staff delivering high quality care in both the hospital and community settings.
  • You will be expected to visit patients in the acute setting to undertake a review of their COPD, where appropriate suggest amended plans of care, support early supported discharge.
  • You will undertake home visits to assess patients in their own homes to determine if it is appropriate to admit them to COPD virtual ward and provide support at home to prevent hospital admission.
  • You will undertake supportive home visits for those patients recently diagnosed with lung cancer.
  • In addition to being a qualified nurse, the successful candidate will also be able to demonstrate excellent computer/ IT skills to include Microsoft Office Package, and be enthusiastic about the field of smoking cessation and health promotion.
  • Experience in clinical audit and data collection is desirable. You must be able to communicate effectively at all levels, have an understanding of managing your own caseload, demonstrate effective teamwork and participate in ongoing audit programmes.
