Job summary
An excellent opportunity has arisen for a motivated and enthusiastic Occupational Therapists to join our Urgent Response and Hospital and Care at home Team. You will be working as a Specialist Band 6 Occupational Therapist. The team will work across the Salford Neighborhoods, whilst assessing and treating patients in their own homes.
We are looking to expand our current team looking for experienced Band 6 Occupational Therapists to join our service. You should have significant post registration experience including working in a community and / or rehabilitation setting, covering a wide variety of clinical areas. You will be a excellent team player and also possess excellent communication skills and must be able to work on your own initiative to organise and prioritise your own caseload and balance this with the demands of this diverse and varied role.
The Service operates over 7 days, currently covering the hours of 8am to 10pm.
It is essential that the post holder is be able to fulfil the travel requirements of the role and vital that they hold a full UK driving license and have access to a vehicle.
Main duties of the job
To assess and treat adults who who are experiencing reduced physical ability and require Rehabilitation and Reablement. The post holder will work as a member of the Urgent Response & Hospital and Care at home Team and will be responsible for providing a Community Rehabilitation Occupational Therapy service to people in their own homes (including residential / nursing homes) across Salford. Their day-to-day workload will include:
- Be experienced and skilled in the holistic assessment of patient's needs and the development, implementation and evaluation of individual treatment programmers.
- Support senior members of the team in delivering evidence-based patient care.
- Work across community settings with experience of wide knowledge base.
- Carry a caseload of patients that present with Acute or Chronic conditions.
- Work as an autonomous practitioner being professionally and legally accountable for all aspects of your work, managing own caseload and maintaining patient and own records.
- Work as a member of the multi-disciplinary team, coordinating discharge and taking an active role in case conferences.
- Provide professional guidance and support for band 5 staff, Technicians / Therapy Assistants and students
- Participate in appropriate MDT meetings, IST and training.
- Assess, plan, prioritise and make recommendations with regards to equipment, adaptations etc., as appropriate to the client group. Assess for and advise on patient's care needs and evaluate this.
