A fantastic opportunity has become available to lead our Dementia and Falls Enhanced Care Team.
The post holder will provide professional leadership for the delivery of a high quality, patient centred service for people living with Dementia, their families and/or carers.
In addition the successful candidate will lead the Falls Prevention service to reduce harm and promote patient safety in a newly developed Enhanced Care service for the Northern Care Alliance.
Main duties of the job
The post holder will have specialist knowledge and experience working with people living with Dementia, providing senior leadership to the Enhanced Care Practitioners in the service.
The post holder will work with the Assistant Director of Nursing to develop and implement strategies, protocols, pathways, and action plans, operationally translated to strengthen the management of Enhanced Care Services.
There will be a focus on robust risk management systems to ensure safe, efficient, effective, and timely management of the Enhanced Care agenda. This will include ensuring a robust professional approach to service development and improvement that meets the statutory, regulated requirements as outlined in legislation, guidance, and best practice.

- Completed or working towards Masters qualification in a related subject or equivalent experience in a relevant specialist area.
