The post holder will be responsible for the provision of ongoing assessment of care needs and the development, implementation and evaluation of programmes of specialised care for children with complex health needs within the local community and specialist education provisions.

Working as part of the wider Community Children's Nursing team providing specialist nursing input for children and young people (CYP) in Education establishment both mainstream School and Local Authority School settings. Supporting school staff to ensure safe access to school for Children & Young People (CYP), with highly complex physical health care needs.

It is essential that the post holder is able to fulfil the travel requirements of the role.

Main duties of the job

To plan, implement and evaluate individualised, holistic, and compassionate nursing care for CYP in a person and family centred way.

To provide skilled nursing care.

To undertake pre-assessment prior to multi-disciplinary clinics.

To deliver the school aged immunisation programme.

To promote continence, assessing and monitoring the appropriate use of continence products.

Undertake health needs assessments for those children in a Specialist School setting.

Provide health education sessions for all children providing advice and support around behaviour, toileting, diet and lifestyle issues.

Ensure height and weight measurements are undertaken for all children in reception and year 6.

To liaise regularly with parents and offer home visits where necessary.

To follow child protection policies and procedures and to liaise closely with the Named Nurse Safeguarding.

Undertake CFC Review Health Assessments and attend Statutory CFC Review meetings.

Provide a public health-based service that contributes to improving the health of children and their families with complex physical health needs in Salford through partnership working.

To ensure effective delivery of the Healthy Child programme, National Childhood Measurement Programme and Immunisation programme as define by DH. Being an autonomous practitioner, addressing health issues raised where appropriate.
