Job summary

We are looking for a versatile, motivated and enthusiastic pharmacy technician to act as a key member of the medicines optimisation team withinCommunity Services . The successful applicant will participate in the development of a range of medicines optimisation activities includingmedication history taking and medicines reconciliation within Intermediate care. They willwork at Killelea House and across the Bury Intermediate Tier including IMC at home & Bury Rapid Response.

Main duties of the job

Duties will include supporting the pharmacists within the intermediate care tier (IMT) to provide medicines management services. To work under the supervision of the pharmacist to assist in the safe, cost-effective delivery of pharmaceutical care to patients, working towards rationalising/optimising their medications whilst under the service.Other duties include providing co-ordinated patient care, working together with other members of the IMT and neighbourhood teams to improve medicines management, identifying any medication issues and working to resolve these across both the home based (IMC@Home & Rapid Response) and bed-based (Killelea House) services and support patients to self-manage their medications and contribute to the avoidance of hospital admission/ re-admission.

  • BTEC Certificate in Pharmaceutical Sciences / NVQ Level 3 (or equivalent)

Knowledge Skills Experience

  • Substantial experience in more than one sector of pharmacy (hospital, PCT / CCG, community)
