Job summary

The post holder will provide evidence based specialist palliative care, advice and support to patients with a diagnosis of cancer or other life limiting illness, and their carers. They will be accountable for their own professional actions without direct supervision. The post holder will deliver high quality palliative care (over a 7 day service) in partnership with nursing, medical and AHP colleagues across departmental and organisational boundaries. They will provide palliative care education on a range of clinical and ethical issues across professional disciplines. The post holder will lead clinical audits in their own area of practice and contribute to the process of Clinical Governance for the Trust.

Main duties of the job

Manage a caseload of patients and their families/carers with specialist palliative care needs, assessing, developing and implementing programmes of care. Provide coordinated and streamlined access to Palliative, End of Life Care and Bereavement services that is able to meet all aspects of End of Life Care and provide consistent outcomes for patients.

Coordinate care for Palliative patients and those approaching the end of their life. Ensure that care is delivered by appropriate services/ teams and that communication and continuity are at the fore of the care plans in place.
