An exciting opportunity has arisen for a Respiratory Specialist Nurse, to work within the Bury Lung Service as part of the wider Northern Care Alliance NHS Foundation Trust.
We are seeking a registered professional with appropriate experience in respiratory care who can demonstrate your commitment, enthusiasm and drive to develop your own clinical experience and team working skills.
You must be able to communicate effectively at all levels, work autonomously and be able to demonstrate effective teamwork. You will provide high quality care in an appropriate and timely manner which is responsive to patients need whilst adhering to recognised policies and guidelines.
The service operates Monday- Friday and covers the following clinical areas
- Home visits
- Pulmonary Rehabilitation / long covid rehabilitation
- Admission avoidance
- Community clinics
- Education and Teaching for the wider Bury community workforce
Access to transport is essential to fulfill community role.
Main duties of the job
To work autonomously but with support from the Bury Lung Service clinical team in the assessment, planning, implementation and evaluation of patient care for patients with COPD/Respiratory disease within the community. Delivering an education package to patients, carers and Primary care colleagues.
You will be supported through regular team meetings, 1-1 sessions, ongoing education with access to a professional team of nurses, AHPs and consultants to provide mentorship and support.
Other main duties include-
Work with other members of the respiratory team to assess, plan and implement care maintaining appropriate documentation.
To deliver a full range of Nursing and psycho-social Care to patients with COPD/respiratory conditions both physical and psychological.
Provide palliative Care and End of Life Care for respiratory patients.
Contribute to the delivery of and promotion of the Pulmonary Rehabilitation programme.
Deliver an educational package to patient, families, carers and significant others.
Carry out specialist assessments of COPD/respiratory patients in both acute and non-acute scenarios.
Organise and triage referrals received into the service.
To promote self management of patients to enable them to feel empowered to manage their chronic respiratory condition.
To develop working relationships and work closely with the wider Primary Care colleagues.
Optimise patient care through collaborative MDT working and appropriate onward referral.
