Job summary

This will be an exciting challenge and will give you the opportunity to play a key-supporting role in the development of the services. You will be working as part of a large multidisciplinary team that includes Nurses, Physiotherapists, Occupational Therapists, Social Workers, Care Support Workers, Housing Officer, Assessment Officers and Administrators, GPs, Advanced Care Practitioners and Paramedics.

Main duties of the job

We are seeking an energetic, dynamic, and enthusiastic individual with strong organisational skills for the role of Flow Coordinator for Salford Royal. You will be working in the Acute Home First Discharge Team in the Hospital.

You will be responsible for liaising with ward and home first hub teams and service teams both within Salford Royal and patients and their families to ensure effective communication is maintained and appropriate and early escalation of discharge planning concerns.You will support the safe and timely discharge/transfer of patients, working with the clinical team to establish patient needs, monitor pathways, plan and help deliver a safe discharge.

You will communicate effectively with Patients, Families and the wider MDT and assist with completion of timely referrals.

You will provide a vital safety role in co-ordination.

You will maintain information of expected discharge dates and discharge criteria pathways, whilst developing knowledge of discharge planning and having experience of working in a multi-disciplinary team.
