Job summary

The Special School Nurse Practitioner will act as a role model in the delivery of health care and promotion to children in an education setting and will provide support to the Team Lead in the effective management of the Service.

The Special School Nurse Practitioner will manage a large case load of children and will promote the holistic health care of the school age child by assessing, implementing and evaluating planned programmes of care thus ensuring that the health needs of any child are met to ensure access to education

The Special School Nurse Practitioner will undertake duties predominantly across 2 school sites and will involve working closely at times with another Special School Nurse and Health Care Support Workers. This role will however at many times require autonomous working on the sites so the post holder will need to be confident in their knowledge and clinical skills of children with a wide range of complex health needs, safeguarding of children and the SEND agenda.

The Special School Nurse Practitioner will also deliver health promotion packages in collaboration with the school health team ensuring that pupils within this caseload have equal access to health promoting messages and access to health information to enable informed choices. Please Note: Post is a Term Time only contract which equates to 30hrs working week

Main duties of the job

Be accountable for the management your own caseload and be a lead professional providing specialist knowledge, advice and support for children and young people with complex and additional health needs.

Formulate, implement, assess and review individual care plans

Develop planned programmes of care to promote health gains and maximise independence.

Work collaboratively with the Education Team, partners and the wider MDT Team to ensure health needs are met thus enabling the pupil access to education.

Be responsible for the safe administration and documentation of medicines

Support children, young people, parents and carers to make informed choices about health-related issues and deliver health education/promotion .

To act as a child's/families advocate in delivering the highest standard of care, ensuring all health care needs are met.

To implement and evaluate evidence-based training programmes for school staff, carers, other professionals, and the multi-disciplinary team and assessing against core competencies and maintaining clear training records as evidence

To work collaboratively with all stakeholders in the transition of children and young people through the education system through to adult services.

To provide health information for the completion of EHCP's annually and undertake RHA assessments.

Proactively safeguard the welfare of children and actively adhere to safeguarding procedures and policies
