Job summary
The role of the Patient Flow Tracker is to ensure tracking and flow support is provided on a daily basis to the Urgent Treatment Centre team to prevent unnecessary delays and improve the patient journey.
A key element of this role is to ensure accurate and timely data entry onto Symphony, and other systems which reflects the patient journey in real time to support the Urgent Treatment Centre in achieving required targets.
The Patient Flow Tracker will escalate potential delays to the relevant team in a timely manner.
The Patient Flow Tracker will work closely with all multi-disciplinary teams (Clinicians, Nursing staff, Therapy staff, Managers, Bed Managers, Wards and Departments, Rapid Response, Local Authorities, PTS and Primary Care Services etc) ensuring the quality indicators are met.
Main duties of the job
Track all patients through their journey within the Urgent Treatment Centre.
In collaboration with Shift Co-ordinator be proactive in identifying all patients who may breach the quality indicators including the emergency 4 hour.
In order to achieve the 4 hour standard cascade/expedite potential delays to the NIC/Co-ordinator.
Liaise with the Manager of the Day and Ward staff regarding bed requirements.
Contact specialities under the direction of the co-ordinator, to facilitate timely speciality response - escalating any issues to the Co-ordinator and/or Nurse in Charge.
Attend UTC Safety Huddle.
Promote the usage of Same Day Emergency Care Unit (SDEC), Minor Eye Care Service and Primary Care appointments.
Liaise with internal and external supporting specialties, departments and services to facilitate movement of patients within the 4 hours
Deal with any telephone enquiries promptly and professionally
Facilitate the booking of patient transport
Liaise with Estates/Path Lab as required
Liaise with IM&T as required, using the appropriate escalation procedure
Liaise with Security and Portering services
Provide reports where requested
Scan patient notes and other relevant documentation prior to transfer/admission using all necessary systems (Symphony/EMIS Tracker etc.).
