Job summary

To provide professional leadership for nursing ensuring that safe and effective quality of care is delivered within the resources available while ensuring that the care environment is organised to meet the needs of the patient enhancing their experience and supporting staff to meet the Divisional and Trust objectives through:

o The provision of visible, authoritative professional leadership support and direction for nursing and by ensuring standards of practice and care are set and maintained.o Ensuring that safe, high quality clinical services are delivered in line with national policy, Trust/ priorities, available evidence and customer/patient care needs.o Lead programmes of improvement work related to the service and the patient experience

Main duties of the job

Main purpose of the job: To provide clinical leadership for the Integrated Community Nursing Teams and support the wider community health and social care teams ensuring that safe and effective quality care is delivered. Secure and assure the highest standards of clinical care by the provision of clinical leadership and support for staff delivering clinical services, service users and their relatives.

  • You must be a Registered General Nurse
  • You must have a Community Specialist Practitioner qualification
  • You must have sound clinical and operational knowledge of community services including experience of working with care staff and highly complex and vulnerable patients.
  • You must be a compassionate and authentic leader.
  • Enthusiastic and committed to developing skills and knowledge.
  • A team player with the ability to work collaboratively with other disciplines.
  • Car driver with ability and readiness to travel to multiple sites.
