Job summary

An opportunity is available for an experienced qualified Social Worker to join the neurosciences social work team in covering the Community Stroke and Neurosciences Rehabilitation Team (CSNRT). This is a full time permanent post, will suit someone looking for challenge and has a keen interest to support an experienced CSNRT service at St James House and Neuro Social work team based at Salford Royal Hospital.

This post provides social work support to Salford Residents who have suffered a brain injury/trauma/acquired and other long term neurological conditions such as MND to support the person and to complement the community rehab goals. We are looking for an experienced qualified social worker, preferably Level 3, who is keen to join a specialist team, develop skills, knowledge and has a passion for supporting people creatively and compassionately who have experienced a brain injury for example, which is often life changing for the person and their loved ones. Home visits would be essential to this post.

We are dedicated to providing services and help to people living in Salford with long term neurological conditions. We aim to provide those with neuro conditions with a service that puts them at the centre and provide support to the loved ones who are also experiencing life changing circumstances.

We provide regular planned supervision, informal supervision and will look at opportunities to exploreprogression and meet learning and support needs of those working in the NCA

Main duties of the job

  • Providing assessments/Reviews which recognise the strength, goals, and needs of the person being assessed.
  • Attending MDT's, professionals' meetings, discharge planning meetings and best interests' meetings as may be required.
  • Being committed to team working and being part of a supportive specialist team
  • Completing capacity assessments and safeguarding enquiry's as appropriate to the case
  • Providing a social work centred role to MDT's and other meetings ensuring the person with the neurological condition is at the centre of decision making.
  • Supporting the loved ones and carers of the person with the neurological condition who may also be facing life changing circumstances.
  • Ability to provide support to the loved ones of those with neurological conditions, given that the change being faced by all can be sudden, this can be challenging and will require resilience and self-care, making the most of reflection and supervision by the social worker.
  • To advocate and challenge when appropriate in the best interests of the person with the neurological condition.
  • To look at the least restrictive options for care and support
  • Ability to manage a complex caseload and will require good organisational skill
  • To provide assessment of needs in accordance with the Care Act 2014, Mental Capacity Act 2005 and Deprivation of Liberty Safeguards and other legislation associated with statutory role. Provide support and services in line with those assessment
