Job summary
This exciting new role has been created to support the assessment and delivery of hyper acute stroke care. Working as part of the acute stroke assessment team based at Salford Royal NHS Foundation Trust.
Based within the Comprehensive Stroke Centre, the practitioner will spend considerable amounts of time in the Emergency Department, supporting the established stroke team in the assessment of patients admitted via the Greater Manchester stroke pathway.
Successful candidates will be expected to support assessment of patients who may be eligible for hyper acute stroke interventions. Alongside management of transient ischaemic (TIA) attack and stroke mimics.
The practitioner will be an expert in the management of patients following an acute stroke, further training will be offered to support this. As the initial assessor the practitioner must be a competent communicator able to gather a comprehensive history in order to recognise the need for escalation when acute interventions are required.
The Practitioner will provide a high standard of clinical leadership to nurses and medical staff, supporting the delivery of evidence-based, patient-centred care. Knowledge of trust policies and procures will ensure that patient care is delivered at high standards based on best available evidence.
The practitioner will also act as mentor to stroke colleagues as they develop their competencies within the specialism of stroke nursing.
Main duties of the job
- Develop advancing knowledge and practice in stroke care.
- Enhanced skills including:
- Venepuncture/cannulation.
- Undertake initial examination of patients using recognised tools including NIHSS.
- Ability to recognise and assess significant unwell states to ensure timely escalation to senior medical colleagues.
- Management of triage, commencing any immediate treatments ensuring timely access to acute stroke interventions such as IV thrombolysis, supporting access to intra-arterial thrombectomy and management of patients with primary intracerebral haemorrhage.
- Order chest x-rays and CT brain scans once IRMER trained (can be completed upon commencement in post).
- Recognition of stroke mimics.
- Dysphagia screening
- Application of Patient Group Directives / Independent prescribing for those with non-medical prescribing qualification.
- Monitor and maintain health, safety and security of self and others in own work area.
- Contribute to the development of services and staff development.
- Support the ongoing development of the stroke nursing workforce through mentorship and competency assessment.
