Job summary
As an End of Life and Bereavement Specialist Nurse you will within the Swan model of care and offer compassionate and discreet support through the immediate grief journey irrespective of age, cause or place of death. This includes supporting / enabling other Healthcare professionals in offering practical and emotional support through memory making, information and guidance before or after the death of a loved one.
In return you will join a supportive team and be provided with opportunities to develop within the role.
Main duties of the job
Work collaboratively with other professionals and agencies. The Bereavement Service is not a counselling service however offers immediate support to those suddenly bereaved and can refer on or signpost families and carers if this level of intervention is identified. This can involve supporting those families going through the coronial process including those who need to attend an inquest.
Support the Lead Nurse in the development and implementation of relevant policies, procedures, guidelines, protocols and education delivery.
You will be allocated a designated site within the Northern Care Alliance however this will be a rotational post and it is anticipated you will be required to work across the hospital sites as the service activity requires.
