Job summary
A rare opportunity has arisen for an experienced clinician with significant Stroke experience to lead the Community Stroke Team working within the Heywood Middleton and Rochdale Community.
We are looking for an experienced, motivated and well organised individual with highly developed skills within stroke care to join our Community stroke team. You will need to be enthusiastic, motivated and an appropriately experienced practitioner in stroke rehabilitation. You will need to be a registered AHP or Nurse who can demonstrate your commitment, enthusiasm and drive to develop your own clinical, managerial and team working skills within the service.
This is a full time position at 37.5 hours per week but applications from suitably experienced candidates who are seeking a minimum of 30 hours per week will be welcomed.
Main duties of the job
You must be able to communicate effectively at all levels, work autonomously and be able to demonstrate effective leadership. You will provide high quality care in an appropriate and timely manner, across Early Supported Discharge (ESD) and Community pathways, which is responsive to patients need whilst adhering to recognised policies and guidelines.
You will be supported through regular team meetings, 1-1 sessions, on-going education with access to a professional team of peers, nurses and AHPs to provide mentorship and support.
In short we are looking for a committed individual with the following attributes:- Registered Nurse or Allied Health Professional- Extensive previous experience in stroke care.- Evidence of post registration training in stroke rehabilitation such as Bobath- A desire to learn and develop clinical skills and knowledge across all aspects of stroke rehabilitation- Excellent standards of care and a positive attitude to managing change and developing services.- Ability to communicate and work collaboratively with patient's carers and other members of the multi-disciplinary team.
A full UK driving licence and access to a vehicle is essential.
