Job summary
You will be based within Salford Older Adult Community Mental Health Team within Greater Manchester Mental Health NHS Foundation Trust. Our multi-disciplinary team works with people with mental health difficulties and dementia across Salford and are looking for a motivated and passionate Social Worker to join our supportive team.
You will provide assessment for service users and carers, in line with the Care Act (2014). You will act as a Care Co-ordinator in line with the Care Programme Approach (CPA) and Trust policy.
You will be an Approved Mental Health Professional (AMHP) or willing to undertake the relevant training, and will carry out the statutory duties of an AMHP in accordance with the relevant legislation and policies.
You will report to the Team Manager for day to day management. Professional social work supervision will be provided by an Advanced Practitioner in Social Work.
Main duties of the job
- Provide care co-ordination under CPA to individual service users experiencing a range of severe mental health issues. As part of this, provide tailored interventions and individualised care which is strengths based, recovery and outcome focused.
- Establish therapeutic relationships and where appropriate provide support and general advice to service users, carers and families. This will require a good knowledge of mental health conditions including different types of dementia, as well as an understanding of how various factors in later life can impact upon an individuals mental health and well-being.
- Assess the health and social care needs of individuals referred to the older age community mental health team who present with a range of functional and organic mental health issues.
- Act as Enquiry Lead in adult safeguarding cases and work in collaboration with other interested parties in the investigation of abuse or neglect in line with s.42 Care Act, local policy and procedures.
