Job summary

This is an exciting opportunity to join our Renal Home Therapies Team as a Band 6 Nurse, supporting patients living with renal failure who choose home dialysis to manage their treatment safely at home while maintaining their independence and quality of life.

The role is varied and rewarding, combining specialist clinical care with education, patient training, and ongoing support for both peritoneal dialysis and home haemodialysis.

We are looking for a motivated nurse with strong clinical judgement, excellent communication skills, and relevant renal experience, including haemodialysis and peritoneal dialysis. You should be able to balance autonomous working with effective teamwork, be organised and adaptable, and confident supporting patients in both hospital and community settings.

Main duties of the job

As a Band 6 Nurse in Renal Home Therapies, you will manage a caseload of patients receiving home dialysis, providing specialist assessment, education, and ongoing support to promote safe and independent care at home.

A central aspect of the role is teaching patients to perform both peritoneal dialysis and home haemodialysis, enabling them to build the skills and confidence needed to manage their treatment.

You will work both autonomously and collaboratively within the Renal Home Therapies Team and wider MDT to coordinate care, evaluate outcomes, and respond appropriately to changes in patients' clinical condition.

You will act as a primary point of contact through telephone, clinic appointments, and home visits, supporting patients and carers with symptom management, resolving dialysis-related issues, and arranging urgent reviews or admissions when needed.

The role involves clinical assessment, interpretation of results, and initiating or adjusting treatment, while promoting self-management and delivering tailored health education.

You will make key clinical decisions, including assessing suitability for home dialysis, recognising and escalating concerns, adjusting care plans in response to changes in condition or treatment tolerance, and determining when patients require admission, discharge, or referral to other services.
