Job summary

Older Peoples Medicine (OPM) is a specialty that has evolved in recent years to meet the needs of the growing population of people over the age of 80 years old. OPM's 5 remaining wards each have a unique specialist focus within the specialism, these Brundall, Dunston, Earsham, Loddon and Elsing Wards.

A large proportion of our patients are either living with dementia or some form of cognitive impairment, being in an hospital environment can be quite distressing/difficult for our patients. Therefore, as a department we are looking for an individual to support our patients during their stay. This role would work across the OPM wards and with our dementia support team to provide holistic and individualized care, to ensure our patients inpatient stay is not distressing for them and families by providing one to one time with patients and acting as an advocate by providing the information and support to the nursing team.

Main duties of the job

The support worker would have the opportunity to work across all of the OPM wards, and as well as work closely with the dementia support team. The role would involve direct patient care, discharge planning, education sessions/training and being involved in meaningful activities/celebrations across the ward areas. The candidate would be in a fortunate position with being able to share good practice and make positive changes across a large area within the medical division.

  • Communicate effectively with multi-disciplinary teams and other departments to ensure care is well planned and coordinated.
  • Support patients, carers and families in situations of emotional distress relating to dementia care
  • Develop and support activity programs under supervision of trained staff
