We are looking for an enthusiastic, professional and dedicated Band 5 Learning Disability and Autism Practitioner to join our expanding Learning Disability and Autism Team here at Team NNUH

An exciting opportunity has arisen for a Learning Disability and Autism Practitioner to join our expanding Learning Disability and Autism Team. The Learning Disability and Autism Team sits within the Complex Health Hub, a collaborative team working to ensure patients with complex needs receive equitable healthcare and outstanding care.

NNUH prides itself on the care it provides to patients with learning disabilities and/or Autistic people. The Learning Disability and Autism team is committed to supporting people who access our healthcare services, to ensure equitable access to care. We are looking to recruit a Learning Disability and Autism Practitioner to work in a new and exciting role, to maintain high standards of patient care under the direction of the Learning Disability and Autism Specialist Nurses.

Main duties of the job

Main responsibilities:

  • To support the delivery of a high quality, safe and compassionate healthcare service, all staff are expected to act as a role model to others in all aspects of their work and consistently demonstrate NNUH's 'PRIDE' values of People Focused, Respect, Dedication, Integrity and Excellence
  • Carry out assessment and evaluation of care relating to patients with learning disabilities and autistic patients aged 0-25 years
  • Communicate effectively with patients, relatives, carers, significant others, the multi-disciplinary team and support services
  • To demonstrate competency and teach others in the safe and effective care of patients with learning disabilities and autistic patients aged 0-25 years
  • Effectively assess and delegate workload according to staff competencies to meet the needs of patients with learning disabilities and autistic patients
  • To assist in leading and monitoring the performance and work of the multidisciplinary team caring for patients with learning disabilities and autistic patients
  • Actively participate in meetings related to own clinical area, including best interest meetings, discharge planning meetings and pre-hospital planning
  • Identify the learning needs of clinical areas in relation to learning disabilities and autism and contribute to the provision and evaluation of learning and development
  • To occasionally make discretionary clinical decisions within the limits of own professional competency without immediate supervision
