Job summary

The Pharmacy Department have an opportunity to recruit a Specialist Pharmacist for admissions, you will become part of a unique team in a highly patient-focused role to collaborate with colleagues on shaping and developing pharmaceutical practice to further develop and progress your career.

Main duties of the job

We are looking for an enthusiastic and dynamic pharmacist with well-developed interpersonal skills to join our medical admissions pharmacy team. The team consists of highly-specialist and specialist pharmacists, technicians and pharmacy assistants and provides an enhanced 7-day service to our ED and medical admissions wards. As part of the team you will be responsible for the provision of pharmacy services to these areas.

The post holder will work alongside the lead pharmacists to assist in developing the pharmacy service to the ED and Acute Medical Units, working closely with the MDTs and through utilisation of independent prescribing (upon qualification) to optimise use of medicines in patients admitted acutely. You will be involved in supporting good medicines management in conjunction with the pharmacy and nursing teams in these areas. Applicants will be self-motivated, able to work independently and under pressure. The successful candidate must be willing to learn and we would support professional development within this role.

This role is full time, we provide a 7-day service, working shifts (8:00am to 7:15pm) including weekends (maximum 1 in 4) and bank holidays. We would consider alternative flexible working patterns dependent on the applicant's requirements.

  • The ED and the medical and surgical admissions units act as the admissions sites for emergency cases. The post holder as part of the admissions team will provide and help to develop the extended clinical pharmacy services to these units
  • To check all prescriptions to ensure that they are safe, legal and cost effective
  • Where possible to liaise with medical and nursing staff to provide proactive suggestions for drug therapy to provide maximum efficiency and safety
  • To review current drug therapy
  • To discuss findings with relevant medical and nursing staff and make recommendations
  • To take full medicines reconciliation
  • To ensure medication is transcribed onto the electronic prescribing and medical administration (EPMA) system under the departmental transcribing policy
  • The post holder will be expected to undertake any other duties that may be allocated by the Principle Pharmacist Team Leader and which may reasonably be expected to be undertaken by the post holder
